Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: Jiaogulan can nudge your blood sugar downward on its own, and empagliflozin (Jardiance) is already working hard to lower it too. Put them together and those two effects can stack up, so your sugar could dip lower than you'd expect. This isn't about Jiaogulan changing how your body breaks the drug down. It's just two blood-sugar-lowering effects adding together.

On its own, empagliflozin rarely causes true lows, but combined with Jiaogulan you may want to watch for shakiness, sweating, dizziness, or feeling foggy, especially if you also take insulin or a sulfonylurea. The evidence here is fairly limited and the concern is mostly theoretical, so don't panic. Just check with your pharmacist or doctor before adding it, and keep an eye on your glucose readings.

From our pharmacist’s overview on the Empagliflozin page

“Almost everything on this page can nudge blood sugar in someone taking empagliflozin (Jardiance) for type 2 diabetes, heart failure, or kidney protection, and two entries deserve genuine caution before anything else. Siberian cocklebur has caused severe low blood sugar in humans and should not be combined with any glucose-lowering medi…”

About Empagliflozin

What Is Empagliflozin Used For?

Brand name: Jardiance

Jardiance (empagliflozin) tablets are approved for several important uses in adults. It reduces the risk of cardiovascular death and hospitalization in adults with heart failure. It also slows the progression of chronic kidney disease by reducing the risk of serious kidney decline, end-stage kidney disease, cardiovascular death, and hospitalization. In adults with type 2 diabetes and established cardiovascular disease, Jardiance reduces the risk of cardiovascular death.

Jardiance is also used — alongside diet and exercise — to improve blood sugar control in type 2 diabetes in adults and children aged 10 and older. It is not recommended for blood sugar control in type 1 diabetes (it may raise the risk of a dangerous condition called diabetic ketoacidosis) or in type 2 diabetes patients with severely reduced kidney function.
