CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Sildenafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: high-dose quercetin supplements can slow down one of the liver enzymes (CYP2C9) that helps clear sildenafil from your body. If sildenafil hangs around longer, its blood levels can creep up. Sildenafil isn't a prodrug, so more drug in your system simply means a stronger effect, and that can mean more of its usual side effects like headache, flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure.
Keep in mind this is mostly theoretical. The real evidence comes from other CYP2C9 drugs, not sildenafil itself, and food amounts of quercetin are unlikely to matter. Concentrated supplements are the concern. Before pairing them, check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the situation with quercetin and sildenafil. Your body clears sildenafil mainly through a liver enzyme called CYP3A4. Quercetin can nudge that enzyme, and the lab and small human studies we have point in conflicting directions. Most of them suggest quercetin slows CYP3A4, which could let sildenafil build up a bit and possibly make side effects more noticeable, things like headache, flushing, a stuffy nose, dizziness, or a drop in blood pressure.
That said, this is mostly test-tube and animal data, so the real-world effect in people is uncertain and probably small with occasional food-level amounts. Concentrated supplement doses are where I'd pay closer attention. Before pairing them, especially if you take sildenafil for lung pressure (Revatio), check with your pharmacist or doctor.

What Is Sildenafil Used For?
Brand name: Revatio
Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.
Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
