Almotriptan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist for almotriptan (Axert): your liver uses an enzyme called CYP3A4 to help clear it from your body. In test-tube studies, horny goat weed seems to slow that enzyme down. If that also happened in people, almotriptan could hang around longer and hit a bit harder, meaning more of its usual effects, like flushing, tingling, dizziness, drowsiness, or that tight, heavy feeling in the chest or throat that some folks notice with triptans.

The big caveat: this is only lab data and has not been shown in actual humans, so it's more of a theoretical caution than a proven problem. I wouldn't lose sleep over it, but since both can affect blood vessels, it's worth a quick chat with your pharmacist or doctor before combining them.

About Almotriptan

What Is Almotriptan Used For?

Brand names: Axert, Almogran

Almotriptan tablets are used to treat migraine headache attacks — with or without aura — as they happen. They are not for preventing migraines before they start.

In adults, almotriptan tablets treat acute migraine attacks. In adolescents ages 12 to 17, almotriptan tablets treat acute migraine headache pain in those who typically have attacks lasting 4 hours or more when untreated. Note that in adolescents, the effect on migraine-related symptoms such as nausea and light sensitivity was not established in clinical trials. Almotriptan is not indicated for cluster headache or for hemiplegic or basilar migraine.
