Nifedipine falls under ANTIHYPERTENSIVE DRUGS — the group Horny Goat Weed interacts with.
Laboratory research suggests that horny goat weed might have hypotensive effects. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: nifedipine already lowers your blood pressure by relaxing blood vessels. Horny goat weed, in lab and animal studies, looks like it might nudge blood pressure down too. Stack the two and the worry is that your pressure could dip a little too low. That can leave you feeling lightheaded, dizzy, flushed, or wobbly when you stand up quickly.
I want to be honest about the strength of this, though. This concern comes from test tube and animal research, and it has not actually been reported in people. So it's more of a theoretical heads-up than a proven problem. If you're curious about trying the herb, run it by your pharmacist or doctor first, and let them know if you notice any dizziness.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Nifedipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for nifedipine: your liver uses an enzyme called CYP3A4 to break it down. In test-tube studies, horny goat weed seems to slow that same enzyme. If that happened in your body, nifedipine could be cleared more slowly and build up, which might exaggerate its blood-pressure-lowering effect. That could mean more dizziness, flushing, headache, swollen ankles, or a faster heartbeat.
The good news is that this is only theoretical. It has been seen in lab dishes, not in real people, so we don't actually know if it matters at usual supplement doses. Still, since nifedipine controls your blood pressure or chest pain, it's worth a quick chat with your pharmacist or doctor before adding this herb. Don't change anything on your own.

Nifedipine treats vasospastic angina (chest pain caused by a spasm in a heart artery) and chronic stable angina (chest pain that comes on with effort). It can be used for chronic stable angina when other treatments, such as beta blockers or nitrates, haven't worked well enough or can't be tolerated.
Nifedipine extended-release tablets, including Procardia XL, are also used to treat high blood pressure. The nifedipine capsule label covers the angina uses only.
