CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Ivabradine
Ivabradine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Corlanor (ivabradine): your liver uses an enzyme called CYP3A4 to clear it out. In test tube studies, horny goat weed appears to slow that same enzyme down. If that happened in your body too, ivabradine could build up and hit harder than expected. With this drug, that usually means an even slower heart rate, dizziness, tiredness, or those odd visual flashes (called phosphenes) that ivabradine can cause.
The honest part: this is theoretical, based only on lab work, never shown in real people. So I wouldn't panic. Still, because ivabradine directly affects your heart rate, it's worth a quick chat with your pharmacist or doctor before adding this herb rather than guessing.

About Ivabradine
What Is Ivabradine Used For?
Brand names: Procoralan, Coralan, Corlentor, Corlanor
Ivabradine is used for stable, symptomatic chronic heart failure in adults to reduce the risk of being hospitalized for worsening heart failure. It is for adults with a reduced ejection fraction (the heart's pumping strength is low, 35% or less), who are in normal sinus rhythm with a resting heart rate of 70 beats per minute or higher. They must also be on the highest beta-blocker dose they can tolerate, or be unable to take beta-blockers.
Corlanor (the brand) is also approved to treat stable, symptomatic heart failure due to dilated cardiomyopathy (a weakened, enlarged heart muscle) in children 6 months and older who are in sinus rhythm with an elevated heart rate. The generic ivabradine tablet labels list the adult use only.
