Here is the interaction we found between these two, with the evidence behind it.
ANTIHYPERTENSIVE DRUGS
Why this applies to Hydrochlorothiazide
Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Horny Goat Weed interacts with.
Laboratory research suggests that horny goat weed might have hypotensive effects. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: hydrochlorothiazide (often sold as Microzide) is a water pill that lowers your blood pressure by helping you shed extra salt and fluid. Horny goat weed has shown some blood-pressure-lowering activity in lab and animal studies, so the worry is that stacking the two could push your pressure down a little too far.
I want to be honest about how weak this evidence is. This effect has only been seen in test tubes and animals, never reported in actual people, so it's more of a theoretical caution than a proven problem. Still, if you do try both, just be aware of signs of low blood pressure like dizziness, lightheadedness when you stand, or feeling faint. Run it past your pharmacist or doctor first, especially if your pressure already runs low.

From our pharmacist’s overview on the Hydrochlorothiazide page
“Hydrochlorothiazide sits at the strong end of the caution scale for supplement interactions, and the small group of major-rated entries deserves a careful read before everything else on the page. Most of what appears here either adds to the pill's blood-pressure-lowering effect or piles onto its diuretic action, pushing fluid and potas…”

About Horny Goat Weed
What is Horny Goat Weed?
- Erectile dysfunction
- Low libido
- Fatigue and low energy
- Bone health
- Joint and menopausal symptoms
- General 'vitality' or performance
Horny goat weed (Epimedium) is a traditional Chinese herb most often marketed for low libido and erectile problems, but solid human evidence for these uses is lacking. While short-term use seems generally tolerated by some people, quality and safety are not well studied, so talk to your pharmacist or doctor before trying it—especially if you have heart problems or take other medicines.
About Hydrochlorothiazide
What Is Hydrochlorothiazide Used For?
Brand names: Esidrix, Hydro, Microzide, HydroDiuril, Hydro-D, Oretic, Aquazide H, Aquazide, HCTZ
Hydrochlorothiazide treats high blood pressure (hypertension) in adults and children, either alone or combined with other blood pressure medicines. Lowering blood pressure reduces the risk of strokes and heart attacks. The capsule form is indicated specifically for hypertension management.
It also treats edema (fluid buildup) associated with congestive heart failure, liver cirrhosis, and kidney diseases including nephrotic syndrome. The tablet form is additionally indicated for edema caused by corticosteroid and estrogen therapy, as well as conditions such as acute glomerulonephritis and chronic renal failure. Inzirqo (the oral suspension) is indicated for both hypertension and edema in adult and pediatric patients.
