Why this applies to Dextromethorphan
Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for cough syrup with dextromethorphan in it. Some lab studies show that horny goat weed can slow down a liver enzyme called CYP3A4, which is one of the helpers that breaks dextromethorphan down. If that enzyme works more slowly, a bit more of the medicine could hang around in your body, which might mean stronger effects like extra drowsiness, dizziness, or feeling a little foggy.
The important thing to know is that this is only seen in test-tube research, not in actual people, so it's mostly theoretical and probably mild for most folks. Still, if you take both and notice you feel unusually sleepy or off, check in with your pharmacist or doctor before changing anything.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.
