Why this applies to Almotriptan
Almotriptan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea: your body uses a liver enzyme called CYP3A4 to break down almotriptan and clear it out. Lab studies suggest neem leaf extract can slow this same enzyme down. If that happened in your body, almotriptan could hang around longer and feel stronger, which might mean more of its usual side effects like dizziness, drowsiness, dry mouth, tingling, or a flushed, tight feeling in your chest or throat.
I want to keep this in perspective, though. This is only a theoretical concern based on test-tube research, and it has never actually been reported in a real person. Still, since neem itself can be risky to swallow, it's worth running both past your pharmacist or doctor before combining them.

What Is Almotriptan Used For?
Brand names: Axert, Almogran
Almotriptan is used to stop a migraine attack once it has started. It is for adults with a history of migraine, with or without aura (visual or sensory warning signs).
It is also used for migraine headache pain in adolescents ages 12 to 17 whose attacks usually last 4 hours or more when untreated. It has not been shown to help migraine-related symptoms such as nausea or sensitivity to light and sound in this age group. It is not for preventing migraines or for cluster headache.
