Estradiol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Your body uses a liver enzyme called CYP1A2 to help clear estradiol. In test-tube studies, neem leaf extract seems to slow that enzyme down. If it does the same thing in people, estradiol could clear a little more slowly and build up, which might nudge up estrogen-related effects like breast tenderness, bloating, nausea, headaches, or spotting.
I want to be honest with you though: this is only seen in lab studies, not in real patients, so it's mostly theoretical. With vaginal estradiol especially, very little reaches your bloodstream, so any effect is likely small. Still, before you pair neem supplements with your estradiol, it's worth a quick chat with your pharmacist or doctor.

In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with neem and estradiol: neem has a pump in your body called P-glycoprotein, which helps move certain drugs along and clear them out. In lab dishes, neem leaf extract seems to slow this pump down. If that happened in real life, a bit more estradiol could stick around, which in theory might nudge up estrogen-related effects like breast tenderness, bloating, nausea, headaches, or spotting.
I want to be honest, though: this is based only on test-tube research, and it has never been reported in actual people. It's a theoretical concern, not a proven problem. Vaginal estradiol also delivers fairly low amounts to the bloodstream. If you'd like peace of mind, just run your neem product past your pharmacist or doctor.
