P-GLYCOPROTEIN SUBSTRATES

Fexofenadine, Pseudoephedrine falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the deal with Allegra-D and neem. The fexofenadine part of this combo relies on a little cellular pump called P-glycoprotein to move it in and out of your body. In lab and animal studies (not in real people yet), neem leaf extract appears to slow that pump down. If that happened in your body, more fexofenadine could linger, which could mean a bit more drowsiness, headache, or dry mouth than usual.
Honestly, this is theoretical and hasn't been seen in humans, so I wouldn't lose sleep over it. The pseudoephedrine side (the decongestant) isn't really affected here. Food-level neem is unlikely to matter; concentrated supplements are the bigger question. If you take neem regularly, just give your pharmacist or doctor a heads-up.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the situation with Allegra-D, which pairs fexofenadine (an antihistamine) with pseudoephedrine (a decongestant). In test-tube studies, neem leaf extract seems to slow down a liver enzyme called CYP3A4. The thinking is that if that enzyme is blocked, drugs it helps clear could build up a bit and produce stronger effects.
I want to be honest with you though: this is only lab data, and it has never actually been seen in people. On top of that, fexofenadine isn't heavily dependent on CYP3A4, so a big problem is unlikely. If levels did creep up, you might notice more of the usual stuff, like a racing heart, jitteriness, trouble sleeping, or higher blood pressure from the pseudoephedrine side. Just run it by your pharmacist before combining them.
