Hydrocortisone falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist if you take neem alongside hydrocortisone. Your body uses a tiny pump called P-glycoprotein to help move certain medicines around and clear them out. Lab studies (in test tubes and animals, not yet in people) suggest neem leaf extract may slow that pump down. In theory, that could let a bit more hydrocortisone hang around in your system than usual.

If that happened, you might notice more of the typical steroid effects: fluid retention, a puffy face, higher blood sugar, mood or sleep changes, or stomach upset. To be clear though, this is theoretical and has never actually been reported in humans, so I wouldn't lose sleep over it. Food amounts are unlikely to matter; concentrated supplements are the bigger question. Just loop in your pharmacist or doctor before starting neem.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP3A4 to help break down and clear oral hydrocortisone (like Cortef). In test tube studies, neem leaf extract seems to slow that enzyme down. If it does the same in people, hydrocortisone could clear more slowly and build up a bit, which might mean stronger steroid effects, things like fluid retention, higher blood sugar, mood or sleep changes, or stomach upset.

The important part: this is only theoretical, based on lab research, and it has never actually been reported in real patients. So I wouldn't lose sleep over it. Still, if you take hydrocortisone daily and want to add a concentrated neem supplement, run it by your pharmacist or doctor first. A skin cream or mouth rinse is much less of a concern.

Here's the thinking behind pairing neem with hydrocortisone. One job hydrocortisone often does is calm down an overactive immune system, which is helpful in conditions like severe allergies, lupus, or certain types of arthritis. Neem appears to do the opposite in some lab and animal studies, where it seems to stimulate the immune system. So in theory, neem could push against what hydrocortisone is trying to accomplish and make it work a little less well.

I want to be honest about how thin this evidence is. It comes from test tube and animal research, not real people, so we don't actually know if it matters in everyday life. Still, if you're relying on hydrocortisone to keep a condition under control, it's worth a quick chat with your pharmacist or doctor before adding neem.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
