Why this applies to Chlorpheniramine
Chlorpheniramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses a liver enzyme called CYP3A4 to help clear chlorpheniramine. In lab dish studies, neem leaf extract appears to slow that enzyme down. If that were to happen in people, chlorpheniramine could hang around a bit longer and build up, which means stronger antihistamine effects. With this medicine, that usually shows up as more drowsiness, dry mouth, blurry vision, constipation, or feeling foggy and dizzy.
I want to be honest about the evidence, though. This is only a theoretical concern from test-tube work, and it has never actually been reported in real patients. Still, if you take neem regularly (not just the trace amounts in toothpaste), it's worth a quick chat with your pharmacist or doctor before mixing the two.

Chlorpheniramine is used to temporarily relieve common allergy symptoms caused by hay fever (allergic rhinitis) or other upper respiratory allergies. These include runny nose, sneezing, itchy or watery eyes, and itching of the nose or throat.
