Neem and Triamcinolone

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Neem interacts with whole categories of medication. Triamcinolone falls into 2 of those categories, so it can interact with Neem in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

P-GLYCOPROTEIN SUBSTRATES

1 of 2

Why this applies to Triamcinolone

Triamcinolone falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the short version: triamcinolone is a corticosteroid that gets carried around and cleared partly by a transporter called P-glycoprotein. In lab dishes, neem leaf extract seems to slow that transporter down. In theory, if neem blocks it, a bit more triamcinolone could linger in your body, which might nudge up its effects and side effects (things like fluid retention, higher blood sugar, mood changes, or trouble sleeping with longer use).

The important caveat: this is only seen in test-tube and animal studies, and it has never actually been reported in a real person. So I wouldn't lose sleep over it. Still, if you take neem as a concentrated supplement (not just a dab of toothpaste or skin product), it's worth a quick mention to your pharmacist or doctor.

Moderate

IMMUNOSUPPRESSANTS

2 of 2

Triamcinolone also falls under IMMUNOSUPPRESSANTS.

Theoretically, neem might decrease the effectiveness of immunosuppressants.
Animal research suggests that neem might have immunostimulant effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the idea behind this one. Triamcinolone is a corticosteroid that works partly by calming down an overactive immune system, which is why it helps with things like arthritis flares, severe allergies, asthma, and certain skin or bowel conditions. Neem, on the other hand, has shown signs in animal and lab studies of revving the immune system up a bit. If that effect carried over to people, it could in theory work against what triamcinolone is trying to do, possibly making it less effective at controlling your symptoms.

I want to be honest though: this is theoretical and based on early research, not solid human studies, so I wouldn't lose sleep over it. Still, before adding a neem supplement, run it by your pharmacist or doctor, especially since concentrated neem can be harsh when swallowed.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.

About Triamcinolone

What Is Triamcinolone Used For?

Brand names: Aristocort, Kenacort, Adcortyl Intra-articular / Intradermal, Kenalog Intra-articular / Intramuscular, Adcortyl in Orabase, Triesence

Triamcinolone is available in many forms — injections, creams, ointments, nasal sprays, dental pastes, and eye injections — each approved for specific uses. Injectable forms given into muscle are used when oral steroids aren't an option, covering severe allergies (like asthma flares, severe skin reactions, drug reactions, and serum sickness), serious skin diseases, certain endocrine (hormonal) disorders, and more.

Injections given directly into a joint (intra-articular) or soft tissue help with conditions like rheumatoid arthritis, gout flares, bursitis, and tendon inflammation. Injections into skin lesions (intralesional) treat conditions such as keloids, alopecia areata, and psoriatic plaques. Triesence (an ophthalmic injection) is specifically indicated for eye conditions like uveitis and temporal arteritis, and for visualization during eye surgery. Topical creams, ointments, lotions, and sprays relieve itching and inflammation from corticosteroid-responsive skin conditions. Dental paste forms (Oralone, Kourzeq) treat painful mouth sores from injury. Nasal sprays temporarily relieve hay fever and upper respiratory allergy symptoms such as runny nose, sneezing, and congestion.
