Why this applies to Telaprevir

Telaprevir falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist: telaprevir is moved around your body by a transporter called P-glycoprotein, and neem leaf extract may slow that pump down. In a test tube, neem blocked this transporter, which in theory could let a bit more telaprevir hang around in your system. If that happened, you might notice more of its usual side effects, like nausea, an itchy rash, anemia, or stomach upset.

I want to be honest about how shaky this is. The only evidence comes from lab and animal studies, and it has never actually been reported in a real person. Concentrated neem supplements are a bigger question mark than tiny amounts in toothpaste or skin products. Before combining them, please check with your pharmacist or doctor.

Telaprevir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist for Telaprevir (Incivek): your liver uses an enzyme called CYP3A4 to break it down. Neem leaf extract has shown, in lab dish studies only, that it can slow that same enzyme. If that were to happen in real life, telaprevir could build up a bit higher than expected, which might mean more of its usual side effects like nausea, itching, rash, or anemia.

The honest truth is this is theoretical. It has only been seen in test tubes, never reported in actual patients, so I don't want to overstate it. Still, since telaprevir is a serious hepatitis C medicine taken alongside ribavirin and peginterferon, please check with your pharmacist or doctor before adding any neem supplement.

About Telaprevir

What Is Telaprevir Used For?

Brand name: Incivek

  • Telaprevir is used along with two other medications (ribavirin [Copegus, Rebetol] and peginterferon alfa [Pegasys]) to treat chronic hepatitis C (an ongoing viral infection that damages the liver) in people who have not yet been treated for this condition or whose condition could not successfully be treated with ribavirin and peginterferon alfa alone. Telaprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Telaprevir may not prevent the spread of hepatitis C to other people.
