Why this applies to Apomorphine
Apomorphine falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with neem and your Apokyn (apomorphine) shots. Apomorphine isn't a prodrug, so it works as soon as it's injected. The concern is that neem leaf extract may block a transporter called P-glycoprotein, which helps move certain drugs out of the body. If that transporter slows down, apomorphine could build up a bit more than usual.
If levels climb, you might notice more of apomorphine's usual side effects, like nausea, dizziness, lightheadedness when standing, drowsiness, or yawning. That said, this is based only on lab research, not real people, so it's a theoretical worry. Still, since apomorphine can drop your blood pressure, it's worth a quick chat with your doctor or pharmacist before adding neem.

About Apomorphine Injection
What Is Apomorphine Injection Used For?

Apomorphine injection is used to treat acute, intermittent episodes of hypomobility — often called 'off' episodes — in people with advanced Parkinson's disease. These include both predictable 'end-of-dose wearing off' and unpredictable 'on/off' episodes, when Parkinson's symptoms suddenly return or worsen between doses of other medications.
It is typically used alongside other Parkinson's medications, not as a standalone treatment. Apokyn is given as an as-needed subcutaneous (under-the-skin) injection. Onapgo is administered as a continuous subcutaneous infusion with the option of supplemental doses.
