dabigatran falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist for Pradaxa (dabigatran). Your body relies on a little cellular pump called P-glycoprotein to help move dabigatran along, including clearing it through your kidneys. In lab dish studies, neem leaf extract appears to slow that pump down. If that happened in a person, more dabigatran could linger in the bloodstream, which mainly raises the risk of bleeding, things like easy bruising, nosebleeds, bleeding gums, or rarely something more serious.

I want to be honest about how soft this evidence is. So far this is only test-tube data, and it has never been reported in real people. Still, because dabigatran is a blood thinner where small shifts matter, I'd run any neem product by your pharmacist or doctor before starting it.

About Dabigatran

What Is Dabigatran Used For?

Brand names: Pradaxa, Pradax

Dabigatran etexilate capsules are used in adults to reduce the risk of stroke and dangerous blood clots in people with non-valvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). They are also used to treat deep venous thrombosis (DVT — a blood clot in a deep vein) and pulmonary embolism (PE — a blood clot in the lungs), to lower the risk of those clots coming back, and to prevent DVT and PE in adults who have had hip replacement surgery.

In children ages 8 to under 18, dabigatran etexilate capsules are used to treat venous thromboembolic events (VTE — blood clots in the veins) and to reduce the risk of those clots recurring. Pradaxa Oral Pellets — a separate formulation — are indicated for the treatment and prevention of recurrent VTE in younger children ages 3 months to under 12 years.
