Trandolapril, Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.

Tarka combines two blood pressure medicines: trandolapril (an ACE inhibitor) and verapamil (a calcium channel blocker). The concern here is that, in lab tests only, neem leaf extract appears to slow down a liver enzyme called CYP1A2. When an enzyme that helps clear a drug is blocked, that drug can build up a bit and its effects may get stronger.

If that happened with this combo, you might see more pronounced effects like dizziness, lightheadedness when standing, a slower heart rate, swollen ankles, or constipation (a common verapamil complaint). That said, this is purely theoretical, based on test-tube work, and has not been reported in real people. Verapamil is actually handled mostly by a different enzyme (CYP3A4), so CYP1A2 may matter less here. Still, since blood pressure changes can sneak up on you, please check with your pharmacist or doctor before adding neem.

Tarka combines two medicines, and the verapamil part is moved around your body by a transporter called P-glycoprotein. In lab dishes, neem leaf extract seems to slow that transporter down. If the same thing happened in people, a bit more verapamil could stay in your system, which might nudge its effects up. With this combo, that could mean lower blood pressure, a slower heart rate, dizziness, tiredness, or constipation.

Here's the honest part: this is only seen in test-tube and animal studies, and it has never been reported in real patients. So I wouldn't lose sleep over it. Still, since both blood pressure and heart rate matter, please check with your pharmacist or doctor before adding a concentrated neem supplement.
