Why this applies to Ivabradine
Ivabradine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your body clears ivabradine using an enzyme called CYP3A4. In lab dish studies, neem leaf extract seems to slow that enzyme down. If that happened in real life, ivabradine could build up a bit more than usual, which can lead to stronger effects like a slower heart rate, dizziness, lightheadedness, or those temporary brightness-in-vision episodes (called phosphenes) that ivabradine is known for.
Here's the important part: this is only a theoretical concern from test-tube research, and it has not been reported in actual people. Still, ivabradine is sensitive to CYP3A4 changes, so I wouldn't combine it with concentrated neem supplements without checking first. Please run it by your pharmacist or doctor before starting neem.

Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. About Ivabradine
What Is Ivabradine Used For?
Brand names: Procoralan, Coralan, Corlentor, Corlanor
Ivabradine (including the brand Corlanor) is used in adults with stable, symptomatic chronic heart failure with reduced pumping function to reduce the risk of being hospitalized for worsening heart failure. It is prescribed when the heart is beating too fast at rest and the patient is either already on the highest tolerated dose of a beta-blocker or cannot take one.
Corlanor is also approved to treat stable, symptomatic heart failure caused by dilated cardiomyopathy (an enlarged, weakened heart muscle) in children ages 6 months and older who have an elevated heart rate.
