Why this applies to Cyclosporine

Cyclosporine falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist with cyclosporine and neem. Your body uses a little cellular pump called P-glycoprotein to help move cyclosporine around and clear it out. Lab tests (done in test tubes and animals, not people) suggest neem leaf extract may slow that pump down. If that happened in real life, cyclosporine could build up a bit higher than expected.

Why care? Cyclosporine is a narrow-margin drug, so even small bumps in its level can matter. More cyclosporine can mean kidney strain, higher blood pressure, headaches, tremor, or swollen gums. The good news: this has never actually been reported in humans, so it stays theoretical for now. Still, please check with your pharmacist or transplant team before adding neem.

Here's the thing to keep in mind with neem and cyclosporine. Your body relies on an enzyme called CYP3A4 to break down cyclosporine and keep its levels in a safe window. Lab studies suggest a concentrated neem leaf extract can slow that enzyme down. If that happened in real life, cyclosporine could build up in your blood, which may raise the odds of side effects like kidney strain, high blood pressure, tremor, headache, or swollen gums.

I want to be honest with you though: this is theoretical, based only on test-tube research, and it hasn't actually been reported in people. Still, cyclosporine is a drug where even small level changes matter, so please loop in your pharmacist or transplant team before adding any neem product.

Here's the concern with pairing neem and cyclosporine. Cyclosporine works by calming down your immune system, which is exactly what keeps a transplanted organ safe or keeps conditions like rheumatoid arthritis and psoriasis under control. Neem, on the other hand, has shown signs in animal studies of revving the immune system up. If that happened in a person, it could push against what cyclosporine is trying to do and weaken its effect.

For someone protecting a transplant, even a small drop in protection is a big deal, so this is worth taking seriously. That said, this is only theoretical, based on lab and animal research, not solid human studies. Please don't start neem supplements without first checking with your transplant team or pharmacist.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
