Why this applies to Buprenorphine

Buprenorphine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to help clear buprenorphine. In test-tube studies, neem leaf extract seems to slow that enzyme down. If that happened in real life, buprenorphine could build up a bit more than usual, which might mean stronger or longer-lasting effects like drowsiness, dizziness, nausea, constipation, or slowed breathing.

The important part: this is only a theory based on lab work, and it has never actually been reported in people taking neem. So I wouldn't lose sleep over it, but I also wouldn't ignore it. Since buprenorphine is a serious medication, please check with your pharmacist or prescriber before adding neem, and don't change your dose on your own.
