Neem and Vericiguat

Here is the interaction we found between these two, with the evidence behind it.

Moderate

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Vericiguat

Vericiguat falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body uses a tiny pump called P-glycoprotein to help move vericiguat (Verquvo) around and clear some of it. In test-tube studies, neem leaf extract seems to slow that pump down. If the pump works slower, a bit more vericiguat could stay in your system, which in theory might add to its blood-vessel-relaxing effect.

For vericiguat, that could mean more low blood pressure, dizziness, or lightheadedness, especially when you stand up. The honest part: this is based only on lab research, not real people, so we don't actually know it happens in humans. Still, since vericiguat already affects blood pressure, I'd loop in your pharmacist or doctor before adding neem rather than guessing.

About Neem

What is Neem?

Neem
  • Skin conditions
  • Dental and gum health
  • Antimicrobial and antiparasitic
  • Digestive upset
  • Blood sugar support
  • Insect repellent/pesticide

Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.

About Vericiguat

What Is Vericiguat Used For?

Brand name: Verquvo

Verquvo (vericiguat) is used to reduce the risk of cardiovascular death and hospitalization due to heart failure. It is specifically for adults with symptomatic chronic heart failure whose heart pumps with reduced function (ejection fraction less than 45%), following a recent hospitalization for heart failure or a need for IV diuretics given outside the hospital.

Verquvo is taken on top of other standard heart failure treatments — it is not a replacement for them.
