Estrogen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.

Here's the gist with neem and your estrogen. Your body partly clears estrogen through a liver enzyme called CYP1A2. Lab studies suggest neem leaf extract can slow that enzyme down. If that were to happen in real life, your estrogen might clear a little slower and build up slightly, which could nudge up estrogen-related effects like breast tenderness, bloating, nausea, headaches, or spotting.

I want to be straight with you though: this is only seen in test tube research, not in actual people, so it's more of a theoretical heads-up than a proven problem. Everyday culinary or dental amounts of neem are unlikely to matter much. Concentrated neem supplements are the bigger question mark. Please don't change anything on your own, just run it past your pharmacist or doctor first.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist for neem and your estrogen. Estrogen is carried around and pumped out of cells partly by a transporter called P-glycoprotein. In lab dish studies, neem leaf extract appears to slow that pump down. If that happened in the body, a bit more estrogen could stick around, which in theory might mean stronger effects or more of the usual estrogen side effects, like breast tenderness, bloating, nausea, or headaches.

The honest part: this is theoretical. It comes only from test-tube and animal research, and it has never actually been reported in real people. So I wouldn't lose sleep over it, but since you're on estrogen long term, it's worth a quick chat with your pharmacist or doctor before starting concentrated neem supplements.
