Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist for your Jentadueto XR (linagliptin plus metformin). Neem leaf extract, in lab dishes only, seems to slow down a cellular pump called P-glycoprotein that helps your kidneys move certain drugs along. If that pump slows, a little more of the medication could linger in your system. For this combo, that could in theory nudge the blood-sugar-lowering effect a bit stronger, meaning more chances of low blood sugar (shakiness, sweating, dizziness) or stomach upset from the metformin side.

I want to be honest though: this has never actually been reported in real people, only in test tubes, so it's a theoretical concern. Food or tea amounts of neem are unlikely to matter, but before starting a concentrated neem supplement, check with your pharmacist or doctor.

Here's the simple version: neem may have a mild blood sugar lowering effect of its own. Your Jentadueto XR already combines two medicines (linagliptin and metformin) that bring your glucose down, so stacking neem on top could push your numbers lower than expected. This isn't about neem changing how your body breaks the drug down. It's just two things working in the same direction.

If that happens, you might notice signs of low blood sugar, like shakiness, sweating, dizziness, a fast heartbeat, or feeling confused or hungry. Honestly, the evidence here is thin (mostly lab and small studies), so I wouldn't panic. But before adding neem, check with your pharmacist or doctor, and keep an eye on your home glucose readings.
