Linagliptin, Metformin Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.

Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the thing to know: with this pairing, your diabetes pill (linagliptin and metformin) is the one doing the influencing, not the other way around. Linagliptin can mildly block the CYP3A4 enzyme, and your body uses that same enzyme to break down modafinil (the active form your liver makes from adrafinil). In theory, that could let modafinil build up a bit, nudging up its effects like jitteriness, a racing heart, trouble sleeping, headache, or higher blood pressure.

I'll be honest, this is a theoretical concern based on how the drugs work, not strong real-world evidence, so it's usually a modest effect. Still, adrafinil is unregulated and tough on the liver, so please run it past your pharmacist or doctor before combining them.
