Acetaminophen, Caffeine, Codeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo pill has three pieces, and the one neem could touch is the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2. In test-tube studies, neem leaf extract seems to slow that enzyme down. If that happened in your body, caffeine might hang around a bit longer, so you could feel more jittery, have a racing heart, or trouble sleeping. The acetaminophen and codeine parts aren't really driven by this pathway, so those shouldn't change much here.
The important part: this is only lab data, and it has never actually been reported in real people. So I wouldn't lose sleep over it, but if you take neem regularly or notice extra caffeine buzz, check in with your pharmacist or doctor.

In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill mixes acetaminophen (Tylenol), caffeine, and codeine. The piece worth watching is codeine. In a lab dish, neem leaf extract slowed down an enzyme called CYP3A4, which is one of the routes your body uses to handle codeine. If that pathway gets blocked a bit, codeine and its byproducts may hang around longer, which could nudge up codeine's usual effects like drowsiness, dizziness, constipation, nausea, or slowed breathing.
Here's the honest part: this is only a test-tube finding, and it has never been reported in actual people. So I wouldn't lose sleep over it, but I also wouldn't combine them on your own. Please run it past your pharmacist or doctor first before adding a neem supplement.
