This combo pill teams up acetaminophen for pain, a little caffeine, and codeine — a narcotic. Here's the wrinkle: lemongrass, in lab and animal studies, seems to slow down a liver enzyme called CYP3A4. That enzyme helps break down codeine. If it's slowed, more codeine could hang around, which theoretically might add to side effects like drowsiness, dizziness, constipation, or slowed breathing.

I want to be honest with you — this is very weak evidence, only from test tubes and animals, not real people. The pinch of lemongrass in your tea or cooking almost certainly won't matter. Concentrated lemongrass supplements or essential oil are where I'd be a bit cautious. Before adding either, just run it by your pharmacist or doctor.

Animal research shows that lemongrass and its constituent citral induce uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation.

This combo pill pairs acetaminophen (Tylenol) with caffeine and codeine for pain. Your liver clears acetaminophen partly through a process called glucuronidation. In lab and animal studies, lemongrass and its main compound citral seem to speed up that pathway. In theory, that could move acetaminophen out of your body a little faster, which might make it slightly less effective for pain or fever.

The honest truth is that this is very early evidence — test tubes and animals, not real people. A cup of lemongrass tea or using it as a cooking herb is unlikely to matter. Concentrated lemongrass supplements or essential oil are the bigger question mark. Before adding a lemongrass supplement, check with your pharmacist or doctor so they can keep an eye on your pain control.
