The piece of this combo pill that matters here is the caffeine. Rauvolfia vomitoria contains yohimbine-like compounds that can rev up the nervous system and push blood pressure higher. Caffeine is also a stimulant, so pairing the two could stack those effects. You might notice a faster or pounding heartbeat, jitteriness, trouble sleeping, anxiety, or a bump in blood pressure.

I'll be honest: this is a theoretical concern based on how these compounds work, not on strong studies in people. Still, Rauvolfia is a potent botanical, not a casual supplement, so the caution is real. The codeine and acetaminophen parts aren't the issue here. Please check with your doctor or pharmacist before combining them, especially if you have heart or blood pressure problems.

This combination pill includes codeine, an opioid that calms the nervous system and can make you drowsy. Rauvolfia vomitoria, which contains reserpine and related alkaloids, also has calming, sedative-type effects. Put them together and those effects could stack up, so you might feel more sleepy, foggy, dizzy, or slowed down than usual. In larger amounts, deeper sedation and slower breathing are the bigger worries with any opioid.

Keep in mind this concern is theoretical, based mainly on how these compounds behave and on animal studies, not strong human data. The caffeine in this product may offset a little of the drowsiness, but it does not cancel the risk. Before pairing the two, please check with your pharmacist or doctor.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the wrinkle with this combo product: the codeine part is a prodrug, meaning your body has to convert it into morphine using a liver enzyme called CYP2D6 before it actually relieves pain. Rauvolfia vomitoria contains a little yohimbine, and lab studies suggest yohimbine can block that same enzyme. If CYP2D6 is slowed down, less codeine gets turned into morphine, so you may notice weaker pain relief than expected.

The good news is this is theoretical, based only on test-tube research, and rauvolfia carries only small amounts of yohimbine. Still, rauvolfia itself is a potent herb that affects blood pressure and mood, so please check with your pharmacist or doctor before pairing it with anything.
