Acetaminophen, Caffeine, Codeine falls under CNS DEPRESSANTS — the group Phellodendron interacts with.
Phellodendron contains berberine. Animal research suggests that berberine may have sedative effects. Theoretically, phellodendron might also have CNS depressants effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill includes codeine, an opioid that can make you drowsy and slow your breathing, especially at higher doses. The concern here is fairly simple: phellodendron contains berberine, which in animal studies showed mild sedative-type effects. So on paper, pairing it with codeine could pile on the sleepiness, leaving you feeling more groggy, foggy, or unsteady than codeine alone would.
I want to be honest though, this is a theoretical worry based on lab and animal data, not on real people taking both together. It's worth a mention rather than alarm. If you're already using a codeine product, just loop in your pharmacist or doctor before adding phellodendron, and go easy until you know how the mix affects you.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Here's the wrinkle with this combo pill: the codeine part is a prodrug. Your body has to use an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves pain. Phellodendron contains berberine, which lab studies suggest can slow down that same enzyme. So in theory, if you block CYP2D6, less codeine gets turned into morphine and you may get less pain relief, not more.
I want to keep this in perspective though. This is a theoretical concern based on lab work, not strong human studies, and the acetaminophen and caffeine pieces aren't really affected. Still, if you rely on this for pain, mention the phellodendron to your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the thing with this combo pill: it's a mix of acetaminophen (Tylenol), caffeine, and codeine. The berberine in Phellodendron may gently block an enzyme called CYP3A4, and codeine partly travels through that pathway. Since codeine is a prodrug that has to be converted into its active form, the bigger and more familiar effect comes from CYP2D6, not CYP3A4, so this particular interaction is mostly theoretical.
If anything shifted, you might notice slightly stronger codeine effects like drowsiness, constipation, or feeling lightheaded, and caffeine could linger a bit longer. The evidence here is weak and based on lab work, so I wouldn't panic. Still, please run it by your pharmacist or doctor before combining them.
