Rifabutin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist with Phellodendron and rifabutin. Your body relies on an enzyme called CYP3A4 to break down rifabutin and clear it out. Phellodendron contains berberine, which can mildly slow that same enzyme down. So in theory, less of the drug gets cleared and a bit more can build up in your system.

If that happened, you might notice more of rifabutin's usual side effects, like nausea, stomach upset, a harmless reddish-brown color to your urine and tears, or in rarer cases joint aches or low blood counts. Honestly though, this is a theoretical concern based on lab work, not real patient reports, so I wouldn't panic. Since you're treating a serious infection, just loop in your pharmacist or doctor before adding this herb.

What Is Rifabutin Used For?

Brand name: Mycobutin

Rifabutin capsules are used to prevent disseminated MAC (Mycobacterium avium complex) disease — a serious bacterial infection that can spread throughout the body — in adults with advanced HIV infection.

It is not approved as a treatment for active tuberculosis (TB), and it should not be used alone for MAC prophylaxis in anyone who has active TB.
