Phellodendron and Diazepam

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Phellodendron interacts with whole categories of medication. Diazepam falls into 3 of those categories, so it can interact with Phellodendron in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CNS DEPRESSANTS

1 of 3

Why this applies to Diazepam

Diazepam falls under CNS DEPRESSANTS — the group Phellodendron interacts with.

Theoretically, phellodendron might increase the sedative effects of CNS depressants.
Phellodendron contains berberine. Animal research suggests that berberine may have sedative effects. Theoretically, phellodendron might also have CNS depressants effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: Diazepam (Valium) already slows down your brain to calm anxiety, ease muscle spasms, and control seizures. Phellodendron contains berberine, which in animal studies seemed to have its own mild calming, sedative effect. So the worry is that stacking the two could leave you more drowsy than you'd expect, with extra grogginess, dizziness, slowed reactions, or trouble concentrating.

I want to be honest about the evidence, though. This concern is theoretical, based on lab and animal work, not solid human studies. For most people it may not amount to much, but it's hard to predict. Before you combine them, run it by your pharmacist or doctor, and don't drive or do anything risky until you know how the pair affects you.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

2 of 3

Diazepam also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2C9.
Phellodendron contains berberine. Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, phellodendron might also inhibit CYP2C9.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist if you're thinking about pairing Phellodendron (which contains berberine) with diazepam (Valium). Berberine may slow down a liver enzyme called CYP2C9, one of the pathways that helps clear diazepam from your body. If that enzyme works slower, diazepam could hang around longer and build up a bit. That might mean stronger or longer-lasting effects like extra drowsiness, dizziness, grogginess, or feeling unsteady on your feet.

I want to be honest, though: this is a theoretical concern based on how these compounds behave, not on real-world cases of problems. So don't panic. Just don't start or stop anything on your own. Check with your pharmacist or doctor first, especially if you already feel sedated on your current dose.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Diazepam also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Valium (diazepam). Your liver uses an enzyme called CYP3A4 to help break this medication down. Phellodendron contains berberine, which lab and early human studies suggest can slow that enzyme. If it's slowed, diazepam may clear a little more slowly and build up in your system.

In plain terms, that could mean more of diazepam's usual effects, like extra drowsiness, dizziness, grogginess, or feeling unsteady on your feet. The good news is this is mostly a theoretical concern based on how berberine behaves, not strong proof in people. Still, since both can make you sleepy, I'd run it past your pharmacist or doctor before combining them rather than starting or stopping anything on your own.

About Phellodendron

What is Phellodendron?

Phellodendron
  • Digestive complaints and diarrhea
  • Inflammation and joint discomfort
  • Stress and weight management (in combination products)
  • Skin conditions (topical, traditional)
  • Antimicrobial support

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. Talk with your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.

About Diazepam

What Is Diazepam Used For?

Brand names: Valium, Valium injection, Diastat AcuDial, Valtoco

Diazepam treats several different problems, depending on the product. Oral diazepam (tablets such as Valium, and oral solution) is used for anxiety disorders, short-term relief of anxiety symptoms, and symptom relief in acute alcohol withdrawal. It is also used alongside other treatments for muscle spasm and, adjunctively, for convulsive disorders (seizure disorders).

Diazepam injection is used for the same anxiety and alcohol withdrawal uses. It is also used before endoscopic procedures, before surgery, and before cardioversion (when given intravenously). It helps with muscle spasm and tetanus, and with status epilepticus (a prolonged, dangerous seizure) as an add-on. Diazepam rectal gel and Valtoco nasal spray are used only for seizure clusters (acute repetitive seizures) in people with epilepsy ages 2 and older.
