Erythromycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Your body clears erythromycin partly through a liver enzyme called CYP3A4. Phellodendron contains berberine, which lab and early human studies suggest can mildly slow that same enzyme down. If that enzyme works slower, erythromycin could linger in your blood at higher levels than usual.

For erythromycin, that mostly means a greater chance of the side effects it's already known for: stomach cramps, nausea, diarrhea, and in some cases changes to your heart's rhythm (a longer QT interval). I want to be honest, though, this concern is theoretical and based on how these compounds behave, not on real cases. Still, since erythromycin already affects heart rhythm, it's worth a quick chat with your pharmacist or doctor before combining the two.

Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. About Erythromycin

What Is Erythromycin Used For?

Brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, Eryc, Ery-Tab, Rythro Enca, Erythro E-C, Erythro Base, Ilosone, Ilotycin

Erythromycin treats infections caused by susceptible bacteria. The oral products (delayed-release tablets, tablets and suspensions) are labeled for mild to moderate upper respiratory tract infections caused by Streptococcus pyogenes, Streptococcus pneumoniae and Haemophilus influenzae. For H. influenzae, it is used together with adequate doses of sulfonamides.

Erythromycin lactobionate injection (Erythrocin Lactobionate and other lactobionate products) is used for similar infections, including lower respiratory tract infections, when oral treatment is not possible or when the infection needs high blood levels right away. Topical erythromycin 2% solution, Ery 2% pads and Spotex gel are used on the skin to treat acne vulgaris.
