Why this applies to Quinidine
Quinidine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for quinidine. Your body relies on a liver enzyme called CYP3A4 to help break this medicine down. Phellodendron contains berberine, which lab and early human studies suggest can mildly slow that enzyme. In theory, that could let quinidine build up a bit higher than expected.
That matters because quinidine is a heart rhythm drug with a narrow safety window, so even small shifts can count. Extra quinidine could mean more side effects like nausea, dizziness, ringing in the ears, or, in rare cases, changes to your heart rhythm itself. To be clear, this is a theoretical concern based on how berberine behaves, not proven harm in people. Still, please check with your pharmacist or doctor before combining them.

What Is Quinidine Used For?
Brand names: Quinaglute Duratabs, Quinaglute, Duraquin, Cardioquin, Cin-Quin, Quinora, Lin-Qin, Quinova, Quinidex Extentabs
Quinidine is used to treat certain abnormal heart rhythms. It can help restore normal rhythm in symptomatic atrial fibrillation/flutter, and it can lengthen the time between episodes in some high-risk patients. It is also used for recurrent, life-threatening ventricular arrhythmias such as sustained ventricular tachycardia.
Quinidine Sulfate tablets are also approved to treat life-threatening Plasmodium falciparum malaria. The Quinidine Gluconate extended-release tablet label does not list this use.
