Verapamil falls under ANTIHYPERTENSIVE DRUGS — the group Phellodendron interacts with.
Phellodendron contains berberine. Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. Theoretically, phellodendron might also reduce blood pressure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Phellodendron contains berberine, a plant compound that may gently lower blood pressure on its own. Verapamil already relaxes your blood vessels and slows your heart rate, so pairing the two could nudge your blood pressure (and possibly your heart rate) down a little further than the verapamil alone.
If that happens, you might feel lightheaded, dizzy, unusually tired, or notice a slow pulse, especially when standing up quickly. I want to be honest, though: this concern is theoretical, based on how berberine behaves in lab and animal studies rather than proven harm with verapamil in people. It's not a reason to panic, but it's worth a heads-up to your pharmacist or doctor before you start phellodendron, and a few extra blood pressure checks at home are a smart idea.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for verapamil and Phellodendron. Your body relies on a liver enzyme called CYP3A4 to break down verapamil. The berberine in Phellodendron may slow that enzyme down a bit, so in theory more verapamil could linger in your bloodstream than usual.
If that happens, you might notice stronger effects from the drug, things like a slower heart rate, dizziness, lightheadedness, swollen ankles, or constipation. The good news is this concern comes from lab and early research, not real cases of people getting hurt, so it's more of a 'be aware' than a 'be alarmed.' Since verapamil affects your heart rhythm and blood pressure, I'd still check with your pharmacist or doctor before pairing the two, rather than changing anything on your own.
