Cyclosporine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist: your body clears cyclosporine mainly through a liver enzyme called CYP3A4. The berberine in phellodendron may slow that enzyme down a bit. If cyclosporine isn't broken down as quickly, more of it can linger in your blood, which can nudge your levels higher than expected.

Cyclosporine is a narrow-therapeutic-index drug, so even small bumps matter. Higher levels can mean more strain on your kidneys, higher blood pressure, tremors, headaches, or swollen gums. To be fair, this concern is theoretical and based mostly on lab work, not strong human studies. Still, because transplant and immune dosing is so precise, please don't add phellodendron without checking with your transplant team or pharmacist first.

Theoretically, phellodendron might increase blood levels of cyclosporine.
Phellodendron contains berberine. Preliminary clinical research shows that berberine can reduce metabolism of cyclosporine and increase serum levels, likely through inhibition of cytochrome P450 3A4 (CYP3A4), which metabolizes cyclosporine. Theoretically, phellodendron might also reduce the metabolism of cyclosporine.

Here's the gist for cyclosporine. Your body relies on a liver enzyme called CYP3A4 to break this medication down. Phellodendron contains berberine, which can slow that enzyme. If it gets slowed, cyclosporine can build up in your blood and push your levels higher than your transplant or arthritis team intended.

Why that matters: cyclosporine is a narrow-therapeutic-index drug, so even modest level changes count. Higher levels can mean more kidney strain, high blood pressure, tremor, or gum overgrowth. The good news is this is mostly a theoretical concern based on how the drug is handled, not strong proof. Still, please loop in your pharmacist or doctor before combining them, since they may want to check your blood levels.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
