Siltuximab falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the good news on this particular pairing: this warning is built around the idea that the berberine in phellodendron can gently slow down a liver enzyme called CYP3A4, which would let drugs handled by that enzyme build up. The thing is, Siltuximab (Sylvant) doesn't really work that way. It's a large monoclonal antibody, a protein-based medicine your body breaks down like other proteins, not something CYP3A4 clears. So the direct mechanism in this alert doesn't fit it well, and the listing itself is only theoretical.

That said, Siltuximab is a serious medication for a serious condition, and adding any herbal product deserves a quick check. Please run phellodendron by your oncology pharmacist or doctor before starting it.

What Is Siltuximab Injection Used For?

Brand name: Sylvant

Sylvant (siltuximab) injection is approved to treat multicentric Castleman's disease (MCD) — a rare disorder involving abnormal lymph node growth driven by an overactive immune signal called IL-6. It is specifically approved for patients who test negative for both HIV and HHV-8 (human herpesvirus-8).

Sylvant has not been studied in patients with MCD who are HIV-positive or HHV-8-positive, because it does not block the form of IL-6 produced by those viruses.
