Phellodendron and Aspirin, Caffeine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Phellodendron interacts with whole categories of medication. Aspirin, Caffeine falls into 2 of those categories, so it can interact with Phellodendron in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 2

Why this applies to Aspirin, Caffeine

Aspirin, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the honest picture with Anacin, which pairs aspirin with caffeine. Phellodendron contains berberine, and lab studies suggest it can mildly slow down a liver enzyme called CYP3A4. The thinking is that if your body clears certain drugs more slowly, those drugs could build up a little. Caffeine is the piece here that's mostly broken down by liver enzymes, so in theory you might feel its effects more strongly, think jitteriness, a racing heart, or trouble sleeping.

That said, this is a theoretical concern based on lab work, not solid human studies, so I wouldn't lose sleep over it (pun intended). Caffeine is actually handled mainly by a different enzyme, so the real-world effect is likely small. Still, it's worth a quick chat with your pharmacist before combining them.

ANTICOAGULANT/ANTIPLATELET DRUGS

2 of 2

Aspirin, Caffeine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, phellodendron might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

The product you're asking about pairs aspirin with caffeine (Anacin and similar). The piece that matters here is the aspirin, because it already works by making your platelets less sticky so they don't clump and form clots. Phellodendron contains berberine, and in lab and animal studies that compound seems to slow platelet clumping too.

So in theory, taking them together could nudge your bleeding risk a little higher. Think easy bruising, nosebleeds, bleeding gums, or a cut that's slow to stop. I want to be honest though: this is theoretical, based only on test-tube and animal work, not real patient outcomes. The caffeine part isn't really involved. It's a reasonable thing to mention to your pharmacist or doctor, especially if you're already prone to bleeding, but no need to panic.

From our pharmacist’s overview on the Aspirin, Caffeine page

“Anacin combines aspirin with caffeine, so supplements can hit it from two directions, and a few of the 347 on this list deserve real respect. The biggest group, over 200 ingredients, can raise bleeding risk on top of aspirin's blood-thinning effect; beer and wine carry major ratings because alcohol plus an aspirin-type drug makes stoma…”

About Phellodendron

What is Phellodendron?

Phellodendron
  • Digestive complaints and diarrhea
  • Inflammation and joint discomfort
  • Stress and weight management (in combination products)
  • Skin conditions (topical, traditional)
  • Antimicrobial support

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. Talk with your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.
