Abemaciclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist for Verzenio (abemaciclib): your body relies on a liver enzyme called CYP3A4 to break this medicine down and clear it. Phellodendron contains berberine, which in lab and early human studies appears to slow that same enzyme. If the enzyme works more slowly, abemaciclib can build up to higher levels than intended.

Abemaciclib is an active drug (not a prodrug), so higher levels usually mean stronger effects and a greater chance of side effects like diarrhea, nausea, fatigue, and drops in your white blood cell counts. I want to be honest, though: this concern is theoretical, based on how berberine behaves in the lab, not proven in people taking this combo. Since abemaciclib is a cancer therapy, please don't start or stop anything on your own. Run it by your oncology team or pharmacist first.

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. About Abemaciclib

What Is Abemaciclib Used For?

Brand name: Verzenio

Verzenio (abemaciclib) is used to treat HR-positive, HER2-negative breast cancer in adults. For early breast cancer at high risk of recurrence, Verzenio is taken alongside endocrine therapy (tamoxifen or an aromatase inhibitor) after surgery to help prevent the cancer from coming back.

For advanced or metastatic breast cancer (cancer that has spread), Verzenio can be used in combination with an aromatase inhibitor as a first hormonal treatment, in combination with fulvestrant after the cancer has progressed on endocrine therapy, or alone after prior endocrine therapy and chemotherapy have stopped working.
