Vitamin E and Abemaciclib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Abemaciclib

Abemaciclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Verzenio (abemaciclib): your body relies on a liver enzyme called CYP3A4 to break it down. There's a theory that high-dose vitamin E may gently speed that enzyme up. If that happened, your body could clear abemaciclib a little faster, which in theory might lower its levels and make this breast cancer treatment less effective. Abemaciclib is not a prodrug, so faster clearance means less drug working, not more.

I want to keep this in perspective. This concern is theoretical, based on lab pharmacology, not real patient cases, and normal food amounts of vitamin E are very unlikely to matter. Concentrated high-dose supplements are the real question. Since abemaciclib is a cancer medicine, please check with your oncology team or pharmacist before adding any vitamin E supplement.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Abemaciclib page

“A daily pill for a common form of advanced breast cancer, abemaciclib (Verzenio) only helps when its blood level stays steady, so this list of 138 supplements deserves genuine care. Nearly all of them act on CYP3A4, the liver enzyme that clears abemaciclib, meaning a supplement can push its level up and intensify side effects, or drag…”

About Vitamin E

What is Vitamin E?

Vitamin E
  • General antioxidant support
  • Skin and hair care
  • Heart health
  • Eye health (age-related conditions)
  • Immune support

Vitamin E is an essential fat-soluble vitamin and antioxidant that most people get in adequate amounts from a normal diet. Supplements can help correct a true deficiency, but high-dose vitamin E has not been clearly proven to prevent heart disease or cancer and may carry risks. Talk to your pharmacist or doctor before taking high doses.

About Abemaciclib

What Is Abemaciclib Used For?

Brand name: Verzenio

Verzenio (abemaciclib) is used to treat adults with hormone receptor (HR)-positive, HER2-negative breast cancer. For early breast cancer at high risk of returning, Verzenio is taken alongside endocrine therapy (tamoxifen or an aromatase inhibitor) after surgery to help prevent recurrence.

For advanced or metastatic breast cancer, Verzenio can be used in three ways: combined with an aromatase inhibitor as a first hormonal treatment; combined with fulvestrant after the cancer has progressed on prior hormone therapy; or on its own after both hormone therapy and prior chemotherapy have stopped working.
