Abemaciclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the situation with your breast cancer medicine, Verzenio (abemaciclib): your liver relies heavily on an enzyme called CYP3A4 to break it down and clear it from your body. Berberine appears to slow that same enzyme down. So if you pair them, abemaciclib could build up higher than intended, which may mean more of its typical side effects, things like diarrhea, fatigue, nausea, low blood counts, or low appetite.

I want to be honest that this is mostly a theoretical concern based on lab work and early studies, not real-world reports of harm with this exact combo. Still, abemaciclib is a serious cancer drug where dosing is carefully balanced, so I wouldn't add berberine on your own. Please check with your oncologist or pharmacist first.

About Abemaciclib

What Is Abemaciclib Used For?

Brand name: Verzenio

Verzenio (abemaciclib) is used to treat HR-positive, HER2-negative breast cancer in adults. For early breast cancer at high risk of recurrence, Verzenio is taken alongside endocrine therapy (tamoxifen or an aromatase inhibitor) after surgery to help prevent the cancer from coming back.

For advanced or metastatic breast cancer (cancer that has spread), Verzenio can be used in combination with an aromatase inhibitor as a first hormonal treatment, in combination with fulvestrant after the cancer has progressed on endocrine therapy, or alone after prior endocrine therapy and chemotherapy have stopped working.
