Saxagliptin, metformin XR falls under ANTIDIABETES DRUGS — the group Phellodendron interacts with.

Theoretically, phellodendron may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Phellodendron contains berberine. Clinical research shows that berberine may lower blood glucose levels. Theoretically, phellodendron might also lower blood glucose levels.

Kombiglyze combines saxagliptin and extended-release metformin, two medicines that already work to bring your blood sugar down. Phellodendron contains berberine, and some research suggests berberine can lower blood sugar on its own. So if you stack them together, the worry is that they could add up and push your numbers too low.

Keep in mind this is a theoretical concern based on how these things work, not proven cases. Still, watch for low blood sugar signs like shakiness, sweating, dizziness, a fast heartbeat, or feeling foggy or hungry. If you're thinking about adding phellodendron, please check with your pharmacist or doctor first, and don't change your diabetes medicine on your own.

Theoretically, phellodendron might increase the therapeutic and adverse effects of metformin.
Phellodendron contains berberine. In vitro and animal studies show that berberine can increase the systemic exposure and half-life of metformin, potentially increasing metformin's effects and side effects. This interaction seems to be most apparent when berberine is administered 2 hours prior to metformin. Taking berberine and metformin at the same time does not appear to increase systemic exposure to metformin. It is unclear if phellodendron might have this same effect.

Your Kombiglyze combines two blood sugar medicines, and the part we're watching here is the metformin. Phellodendron contains berberine, and in lab and animal studies berberine seemed to let more metformin build up in the body and stick around longer. In theory, that could nudge metformin's blood sugar lowering a little stronger, and possibly bring on more of its usual side effects like nausea, an upset stomach, diarrhea, or a metallic taste.

Keep in mind this is theoretical, based on how the compounds behave, not solid human studies, so I wouldn't lose sleep over it. Interestingly, the effect showed up mainly when berberine was taken a couple of hours before metformin, not at the same time. Still, since you're managing diabetes, please check with your pharmacist or doctor before adding phellodendron.
