Trandolapril, Verapamil falls under ANTIHYPERTENSIVE DRUGS — the group Phellodendron interacts with.

Theoretically, phellodendron might have additive effects with antihypertensive drugs.
Phellodendron contains berberine. Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. Theoretically, phellodendron might also reduce blood pressure.

Tarka is a combo pill that already lowers blood pressure two ways: trandolapril (an ACE inhibitor) relaxes your vessels, and verapamil (a calcium channel blocker) eases the heart's workload. Phellodendron contains berberine, which in animal studies and one small human study seemed to nudge blood pressure down a little on its own. So the worry here is simple addition: stacking the herb on top of Tarka could push your numbers lower than intended.

If your pressure drops too far, you might feel lightheaded, dizzy, tired, or unsteady when you stand up. Keep in mind this is a theoretical concern based mostly on lab and limited data, not strong proof. Still, please check with your pharmacist or doctor before adding it, and don't change your Tarka dose on your own.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Your Tarka combines trandolapril (a blood pressure medicine) with verapamil, and that verapamil piece is the one to watch here. Verapamil is cleared from your body mostly by an enzyme called CYP3A4. Phellodendron contains berberine, which lab and early human studies suggest can partly block that same enzyme. If it does, verapamil may clear more slowly and build up in your blood.

In real terms, that could mean more of verapamil's usual effects: lower blood pressure, a slower heart rate, dizziness, tiredness, swollen ankles, or constipation. To be fair, this concern is theoretical and based on lab work, not strong human proof. Still, before adding phellodendron, please check with your pharmacist or doctor first.
