Here's the simple version: amitriptyline already tends to make people drowsy. It's one of the more sedating antidepressants, and on its own it can leave you feeling tired, foggy, or a little unsteady, especially when you first start it. Phellodendron contains berberine, and in animal studies berberine showed some calming, sleepy effects of its own.
Put the two together and the worry is simply extra sedation, more grogginess, trouble concentrating, or feeling off-balance. I want to be honest, though: this is a theoretical concern based on how the plant behaves in the lab, not proven in people. CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Here's the simple version. Your body uses a liver enzyme called CYP2D6 to clear amitriptyline (Elavil) out of your system. The berberine in phellodendron may slow that enzyme down. If that happens, amitriptyline could build up a bit higher than usual, which can mean stronger side effects like drowsiness, dry mouth, constipation, blurry vision, dizziness when you stand, or a faster heartbeat.
I want to be honest about the strength of this, though. It's based on lab work and theory, not solid human studies, so it may not amount to much in real life. Still, since amitriptyline is sensitive to changes, please check with your pharmacist or doctor before pairing the two so they can watch how you feel.

Here's the gist: phellodendron contains berberine, and early research suggests berberine can slow down a liver enzyme called CYP2C9.
