Here's the short version for combo pills like Excedrin (acetaminophen, aspirin, and caffeine). The concern on file is that Phellodendron contains berberine, which may mildly slow down a liver enzyme called CYP3A4. When that enzyme is slowed, drugs it clears can hang around a bit longer. Of the three ingredients here, caffeine is the one most likely to be affected, so you might feel jittery, have trouble sleeping, or notice a faster heartbeat. The pain relievers themselves aren't heavily dependent on this pathway.

Honestly, this is a theoretical interaction based on lab work, not strong human studies, so I wouldn't lose sleep over it. Still, if you take Phellodendron regularly, mention it to your pharmacist or doctor before pairing the two.

The product you're asking about (think Excedrin Migraine) combines acetaminophen, aspirin, and caffeine. The piece that matters here is the aspirin, which already thins the blood a bit by keeping platelets from clumping together. Phellodendron contains berberine, and in lab and animal studies berberine seems to slow platelet clumping too. Stack the two and you could, in theory, nudge your bleeding risk a little higher.

Be honest with yourself about the evidence here: this is based only on test-tube and animal research, not real people, so it's a maybe, not a sure thing. Still, watch for easy bruising, nosebleeds, or bleeding gums. Before combining them, run it by your pharmacist or doctor, especially if you take aspirin daily.
