Sitagliptin, Metformin falls under ANTIDIABETES DRUGS — the group Phellodendron interacts with.

Theoretically, phellodendron may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Phellodendron contains berberine. Clinical research shows that berberine may lower blood glucose levels. Theoretically, phellodendron might also lower blood glucose levels.

Here's the simple version: your Zituvimet combo (sitagliptin plus metformin) already works to bring blood sugar down. Phellodendron contains berberine, and some research suggests berberine can lower blood sugar on its own too. Stack the two together and, in theory, your numbers could dip lower than expected. That's the main concern here, an added blood-sugar-lowering push rather than the herb changing how these drugs are broken down.

What to watch for: shakiness, sweating, dizziness, headache, or feeling suddenly hungry or confused can all signal low blood sugar. Keep in mind this is a theoretical, pharmacology-based caution, not something proven in strong human studies. Still, before adding Phellodendron, check in with your pharmacist or doctor so they can help you monitor your levels.

Theoretically, phellodendron might increase the therapeutic and adverse effects of metformin.
Phellodendron contains berberine. In vitro and animal studies show that berberine can increase the systemic exposure and half-life of metformin, potentially increasing metformin's effects and side effects. This interaction seems to be most apparent when berberine is administered 2 hours prior to metformin. Taking berberine and metformin at the same time does not appear to increase systemic exposure to metformin. It is unclear if phellodendron might have this same effect.

Your Zituvimet combines two blood sugar medicines, and it's the metformin part that matters here. Phellodendron contains berberine, and in lab and animal studies berberine seemed to slow how fast the body clears metformin. That could let metformin build up a bit and act more strongly, which may increase the chance of side effects like nausea, stomach upset, diarrhea, or in some cases low blood sugar (feeling shaky, sweaty, or lightheaded).

Keep in mind this is theoretical. It's based on berberine, not phellodendron directly, and the effect mostly showed up when berberine was taken about 2 hours before metformin, not together. It's a fairly mild concern, but since your diabetes control matters, please check with your pharmacist or doctor before adding this supplement.
