Theoretically, phellodendron may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Phellodendron contains berberine. Clinical research shows that berberine may lower blood glucose levels. Theoretically, phellodendron might also lower blood glucose levels.

Here's the thing to keep in mind if you're taking metformin and thinking about phellodendron: this bark contains berberine, and berberine has been shown in some studies to lower blood sugar on its own. Since metformin is already working to bring your glucose down, stacking the two could push your blood sugar lower than you intend.

If that happens, you might notice signs of low blood sugar (hypoglycemia) like shakiness, sweating, dizziness, a fast heartbeat, or feeling confused. To be fair, this concern is theoretical and based on how these compounds work rather than solid studies on the two together, so it isn't a sure thing. Still, I'd check in with your pharmacist or doctor first, and keep a closer eye on your blood sugar readings if you do use both.

Theoretically, phellodendron might increase the therapeutic and adverse effects of metformin.
Phellodendron contains berberine. In vitro and animal studies show that berberine can increase the systemic exposure and half-life of metformin, potentially increasing metformin's effects and side effects. This interaction seems to be most apparent when berberine is administered 2 hours prior to metformin. Taking berberine and metformin at the same time does not appear to increase systemic exposure to metformin. It is unclear if phellodendron might have this same effect.

Here's the gist: Phellodendron contains berberine, and in lab and animal studies that compound seems to slow how fast your body gets rid of metformin. The result is that more metformin can hang around in your system. That could make metformin work a bit harder than expected, which also means a higher chance of its usual side effects, like stomach upset, nausea, diarrhea, or a metallic taste. Low blood sugar is uncommon with metformin alone, but stronger effects are still worth watching for.

Keep in mind this is theoretical, based on pharmacology and animal data, not solid human studies. Interestingly, the effect mostly showed up when berberine was taken about 2 hours before metformin, not at the same time. Before adding phellodendron, please check with your pharmacist or doctor so they can help you watch your blood sugar.

Metformin — available as tablets, extended-release tablets, and an oral solution — is used to improve blood sugar (glucose) control in people with type 2 diabetes mellitus, always as part of a plan that includes diet and exercise.

Metformin hydrochloride tablets and the oral solution are approved for adults and children 10 years and older. Extended-release tablets (including Fortamet, Glumetza, and Metformin Hydrochloride Extended Release) are approved for adults only. Riomet (oral solution) is approved for adults and children 10 years and older.
