The piece of this combo pill that matters most here is the codeine, an opioid that can slow down your brain and make you sleepy. THC from cannabis can do the same kind of thing. Stack the two together and those calming, sedating effects can add up, so you may feel more drowsy, dizzy, foggy, or slow to react than you'd expect from either one alone. That's a real safety concern for driving or anything that needs full attention.

To be fair, this is a theoretical concern based on how these substances behave, not on hard studies, so it won't hit everyone the same way. Still, if you use cannabis and take this medication, it's worth a quick chat with your pharmacist or doctor first.

Theoretically, THC might decrease the levels and clinical effects of CYP2E1 substrates.
In vitro research shows that cannabis containing THC can induce the activity of CYP2E1, which might increase the metabolism of CYP2E1 substrates. It is unclear if this effect is due to THC, other constituents, or the combination.

Here's the situation with this combo pill, which contains acetaminophen (Tylenol), caffeine, and codeine. The piece that interacts is the acetaminophen. Lab and animal studies suggest THC might speed up an enzyme called CYP2E1, which is one of the pathways your liver uses to handle acetaminophen. In theory, that could shift more acetaminophen down a route that creates a harsh byproduct, which is the same byproduct linked to liver strain in overdoses.

I want to be honest with you though: this is only test-tube and animal evidence, so it's theoretical and far from proven in real people. For most folks taking normal doses it likely won't cause noticeable problems. Still, since acetaminophen and the liver are involved, it's worth a quick chat with your pharmacist or doctor, especially if you drink alcohol or take other acetaminophen products.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Let's keep this one in perspective. The concern on file is that THC might gently slow down a liver enzyme called CYP3A4, which helps process the codeine portion of this combo. If that enzyme were blocked even a little, codeine could hang around longer, theoretically nudging up its effects like drowsiness, dizziness, constipation, or slowed breathing.

Honestly, though, this is mostly a lab-dish idea. In a real human study, oral THC did not meaningfully block CYP3A4, and the database itself rates this as minor and unlikely. The bigger thing to watch is that both THC and codeine can make you sleepy and foggy, so combined they may hit your alertness and driving harder. If you use both, just mention it to your pharmacist or doctor.
