Theoretically, THC might reduce the levels and clinical effects of theophylline.
Smoking cannabis containing THC seems to increase the metabolism of theophylline. It is unclear if this effect is due to THC, other constituents, or the combination.

Here's the gist with theophylline and THC. Theophylline is an active drug (not a prodrug), and your liver clears it out over time. The concern is that smoking cannabis may speed up that clearance, so your theophylline levels could drift lower. If that happens, the medicine might not control your breathing as well, and you could notice more wheezing, coughing, or chest tightness.

I want to be honest about the evidence here. This is mostly theoretical, and it's unclear whether THC itself, the smoke, or both is responsible. Still, theophylline has a narrow safe range, so even modest changes matter. Please loop in your doctor or pharmacist before mixing the two, especially since they may want to check a blood level.

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.

All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
