Why this applies to Quinidine

Quinidine falls under P-GLYCOPROTEIN SUBSTRATES — the group Delta-9-tetrahydrocannabinol (thc) interacts with.

Theoretically, THC might alter levels of drugs that are substrates of P-glycoprotein (P-gp).
Most in vitro research suggests that THC can inhibit P-gp and increase the accumulation of probe compounds by reducing P-gp mediated drug efflux. In vitro studies in kidney cell lines show that a 1-hour exposure to CBD and THC inhibits P-gp. THC may also alter the expression of P-gp, although this effect appears to vary based on duration of exposure. Some in vitro research in lymphoblastoid leukemia cell lines indicates that a 1-hour exposure to cannabinoids does not affect P-gp expression, while a prolonged 72-hour exposure decreases P-gp expression. Other in vitro research in these cell lines shows that a 4-hour exposure to THC and CBD induces P-gp gene expression, while exposure for longer than 4 hours and up to 48 hours does not induce P-gp gene expression.

Here's the gist for you. Quinidine relies on a transporter called P-glycoprotein (P-gp) to help move it out of cells and keep its levels in check. In lab and animal studies, THC appears to block that transporter. If that happened in real life, your body might hold onto more quinidine than usual, which could nudge its levels up and increase the chance of side effects like dizziness, nausea, diarrhea, or in some cases changes to your heart rhythm.

I want to be honest, though: this is theoretical and based only on test-tube and animal research, not human studies. Still, quinidine is a heart rhythm drug with a narrow safety margin, so even small shifts deserve attention. Please loop in your pharmacist or doctor before combining the two.

Quinidine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Here's the gist for quinidine: your liver uses an enzyme called CYP3A4 to help clear it. Lab studies suggest THC might slow that enzyme down a little, which in theory could let quinidine build up a bit. Since quinidine controls heart rhythm and has a narrow safety window, higher levels could mean side effects like dizziness, nausea, ringing in the ears, or, more importantly, changes in your heartbeat.

That said, this is mostly test-tube data, and one human study actually showed oral THC did not block CYP3A4. So the real-world risk looks small and unlikely. Still, because quinidine is a heart medicine where small shifts matter, I'd loop in your pharmacist or doctor before combining the two.

What Is Quinidine Used For?

Brand names: Quinaglute Duratabs, Quinaglute, Duraquin, Cardioquin, Cin-Quin, Quinora, Lin-Qin, Quinova, Quinidex Extentabs

Quinidine is used to help restore a normal heart rhythm in people with atrial fibrillation or atrial flutter (irregular, rapid heart rhythms originating in the upper chambers). It is also used on an ongoing basis to reduce how often those episodes come back, in patients whose episodes are frequent and poorly tolerated — but only after other options have been tried first.

Quinidine sulfate oral tablets and quinidine gluconate extended-release oral tablets are both approved for these heart-rhythm uses. Quinidine sulfate tablets are additionally used as part of an approved treatment regimen for life-threatening Plasmodium falciparum malaria, in combination with quinidine gluconate injection (the central component of that regimen). Both formulations may also be used to suppress recurrent, documented life-threatening ventricular arrhythmias (dangerous rapid rhythms in the lower chambers of the heart), such as sustained ventricular tachycardia.
