Here's the simple version: Glucovance combines glyburide and metformin, and both already work to bring your blood sugar down. Neem may have its own mild blood-sugar-lowering effect, so stacking the two could push your numbers lower than you intend. The glyburide piece especially can cause low blood sugar (hypoglycemia) on its own, so that's the part I'd watch most closely.

If your blood sugar drops too far, you might feel shaky, sweaty, dizzy, confused, or unusually hungry. The strongest evidence here comes from lab and animal work plus a few small human studies, so this isn't a guaranteed problem, but it's worth respecting. Please don't change either one on your own. Loop in your pharmacist or doctor first, and keep an eye on your home readings if you do start neem.

Glyburide, Metformin also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.

Here's the gist with Glucovance, which combines glyburide and metformin. The glyburide half is handled by a liver enzyme called CYP2C9. In lab dish studies, neem leaf extract seemed to slow that enzyme down. If that were to happen in real life, your body might clear glyburide a little slower, letting it build up. Since glyburide already works by lowering blood sugar, more of it could push your sugar too low.

Watch for shakiness, sweating, dizziness, hunger, or confusion, which are classic signs of low blood sugar (hypoglycemia). I want to be honest though: this is only a test-tube finding and hasn't been seen in people, so don't panic. Just give your pharmacist or doctor a heads-up before combining them.
