Acetaminophen, Caffeine, Dihydrocodeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo pill. The piece that interacts with neem is the caffeine, which your body clears using an enzyme called CYP1A2. In lab dishes, neem leaf extract seems to slow that enzyme down. If that held true in people, caffeine could hang around longer and build up, leaving you feeling jittery, restless, sped-up heart rate, or having trouble sleeping. The acetaminophen and dihydrocodeine parts aren't really driving this particular interaction.
Now the honest part: this is only seen in test-tube studies and hasn't been reported in actual patients, so I wouldn't lose sleep over it. Still, if you take concentrated neem supplements and notice extra caffeine-type jitters, mention it to your pharmacist or doctor before changing anything.

In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with this combo pill (acetaminophen, caffeine, and dihydrocodeine). The piece that matters most is the dihydrocodeine. Like its cousin codeine, it's partly a prodrug, meaning your body has to convert it into the stronger active painkiller. Liver enzymes, including CYP3A4, help process it. In test-tube studies only, neem leaf extract appears to slow down CYP3A4. This has never actually been reported in real people, so consider it more of a theoretical heads-up than a proven problem.
If neem did slow this enzyme, levels of dihydrocodeine and the caffeine could drift upward, which might mean more drowsiness, constipation, nausea, or feeling jittery. It's a low-likelihood concern, but since opioids carry real risks, it's worth a quick word with your pharmacist or doctor before pairing them.
